Student Advocacy Board Petition Form

The Student Advocacy Board provides you with an opportunity to present a concern to an impartial group of faculty and staff members. To request the Board's consideration, please complete this form within five (5) business days of the conclusion of any informal attempts to resolve this concern at the departmental level.

Before the Student Advocacy Board will review your petition, you must demonstrate that you have made all attempts to resolve your concern with the department(s) involved, in accordance with their procedures. In the section below, please provide the name(s) of the individual(s) that you have worked with and the dates of any conversations and/or meetings you had about this concern.
